HRT actually shrinks your penis and testicles, witch makes them easier to hide.
But Phil isn't on HRT and he just has a genuinely tiny dick.

In some cases were there is less skin available (like with a tiny dick or a orchiectomy),
they will graph skin from the leg or another body part for vaginoplasty (SRS). But that is rare and most surgeons don't want to do that because of potential risks.
This is the problem a orchiectomy can cause, and this is also why if you get a orchiectomy you have to keep the skin stretched so the skin can be used later on.
This is also why some transgender woman get prosthetic testicles put in during the orchiectomy, so the skin is guaranteed to stay in a useable state, without having to worry about keeping it stretched.

The other problem Phil has is that he is fat.
