Context: Prior to operation Ajax in 1953, the US had approached Iran in secret to negotiate a (very generous) deal to compensate the British for stolen oil assets. Despite almost signing the deal and avoiding the revolution outright, the the deal was refused because it wasn't made out of "real care" for Iranian wants.

There is no reason why Khamenei wouldn't release a speech or some other proof of life if he was still alive; showing the faithful that the supreme leader is still standing would have a powerful morale effect on both regime loyalists and attacking Americans. The fact that the regime has not done this is almost certainly because they cannot.

Iran is a very long time country/culture/government. So it is very likely they have very long term succession plans in place - the Ayatollah position exists and they also have a prime minister and shit.
HOWEVER, Israel is obviously using the same methodology that they used with Hamas in Palestine and assassinating anyone who is a "who" and this will seriously stifle the government "working" as it should and delay any succession plan and also potentially make the government completely dysfunctional to the point it cannot elect a competent leader.
While a lot of people probably think Iran is fucked (and it probably is) the fact they're still launching attacks means there is some structure or communication going on and they still have soldiers loyal to the government.
