It's an edit. In the original, Frank reveals his mother was worried about him getting Stephie pregnant. Frank laughs it off, knowing that Stephie can't get pregnant, but Stephie cries at the reminder of her biology. The URL at the bottom of the image @Iron Jaguar posted should also make it obvious that it's an edit.

Totally, except I'm pretty sure that in the original comic Frank's mother was concerned that Stephie couldn't get pregnant. Either way it's a bizarre thing for the parent of a pre-teen to worry about.

Let's be fair: for a certain kind of tranny it is a little sad that they'll never have children. I'll save my tears for the gaylords and lesbos who reach a certain age and realise that starting a family is something they cannot do. That must hurt. My heart isn't made of stone.
The way that some people are orientated means they can't have kids, and seeing your children grow is one of life's greatest pleasures. The LGBTBBQ contains many people who are basically normal, and one particular milestone in life is out of their reach. It's true.

Still, Buttstuff or not, no junior should be considering pregnancy. @Iron Jaguar did a great edit. Labelle should be kept away from children.

The next comic in the series was much worse.

The one time he features a legitimate argument that normal people make against trannies, his response is totally bonkers.

Any mother in law would need to fight their instinct to not punch the tranny and the son if they heard that about their grandchildren.

I think it would be less disrespectful to tell the mother in law; "listen, your son is a fucking faggot, best you can hope for is for us to adopt if we feel like it".
 
I can see being of the age for your kids to start making their own kids and realizing that you're probably never going to be a grandparent and taking it hard, but people deal with that shit for all kinds of reasons.

Maybe your kid got cancer or had his nuts blown off in Afghanistan or is an unfuckable NEET... there are a hundred reasons.

But for kids this age, it's creepy.
