Anyone who can't clean a toilet has bigger concerns than needing their toilet cleaned. It takes a few minutes to do if you're reasonably able-bodied (ie can bend down for a few minutes) and if you can't do that then you can't take out your garbage. And as bad as a gross toilet is, I'd be much more concerned about the inevitable trash piling up and if someone asks me to either clean their nasty toilet or take out their trash (or possibly pick up some trash) then I'm picking the trash every time.
If I found out that there was someone going around, specifically cleaning toilets as a good deed, then my assumption is that they are a fetishist. Which, considering Anisa's history with piss, is surprisingly plausible.
