The date "07/21/2021" is coming from the "PAYMENT INFORMATION" section where it says "LAST PAYMENT DATE > 07/21/2021", it only reflects the last time a payment was processed in the system. It does not indicate when the ticket was issued.

Also DGGers are currently coping and saying that since the "JUVENILE" checkbox isn't checked, it means she wasn't a minor at the time of the ticket. Which is obviously not true. She clearly was.
After some digging, I found out that the checkbox might not have been checked because Illinois law (705 ILCS 405/5-125) allows traffic violations by minors to be prosecuted in adult traffic court instead of juvenile court. And they might've purposefully left it unchecked to indicate the fact she was prosecuted in adult traffic court, not juvenile court. Plus a checkbox is meaningless, clerical errors happen and her DOB is clearly marked "10/04" (2004-October-??).

Some extra clarity:

"DISPOSED: Withhold Judgment/Supervision – 07/06/2021": Meaning that on 2021-July-06 the court entered a final decision on the ticket but did not immediately record a conviction. Instead, she was placed under supervision, meaning she had to follow certain conditions (like paying fines or attending traffic school). If she successfully completed these conditions, the violation would not appear as a conviction on her record.

So then, the "LAST PAYMENT DATE > 07/21/2021", (2021-July-21), would then be indicative of when she actually paid any possible fines, fees, or court costs associated with the ticket.
