Personally, I’ve only ever watched the anime all the way through. While it still has a lot of creepy moments from the manga, at least the anime has the decency to tone it down a couple notches.
The manga on the other hand, I ended up dropping half way through because I couldn’t take the creepiness anymore. As you said, it’s impossible to separate art from the artist in the context of the manga.
Seeing the page where he described what Nanachi’s butthole smells like what the final straw for me. Normally I drop a series a lot quicker than that, but was only pulled in by the story.
To be honest, it’d be an easy 10/10 if not for the creepy fetishes and pedophilia.

Why is all anime like this now? Surely it wasn't this bad during the 90s? I don't remember child sexual assault being a key feature of Cowboy Bebop or DBZ. Did something change or has it always just been lurking in the shadows waiting for an audience?

The difference between now and then, as far as importation goes, is that this type of shit used to get filtered out or censored.
Full frontal nudity from Dragon Ball was edited out. I remember one segment of Digimon, where an entire sequence of an 11 year old trying on swimsuits got considerably toned down compared to the original.
Honestly, if those shows came out today with the same level of “censorship”, you bet your ass Lolicons would be fuming over this.

Even Cardcaptor Sakura has an arc in the Japanese one where her 11-13yo friend has a relationship with the teacher. It's just cut in the Western dub. I think they reuse a handful of scenes in a total different context but yeah.
And even famous weeb stuff from back in the day like Chobits, the first episode has him waking up his lolibot accidently when he pokes her in the vag where her power button is conveniently located.
And stuff like Green Green which nobody seems to care about nowadays but was on every anime piracy site back in the day is about a load of high schoolers fucking.
Even when the medium was new to the West, it was still there.
I can make some excuses for some of the stuff in Dragonball, like Goku and sleeping Bulma (hes a boy from the mountains who's never seen another human except his grandfather, and he doesn't know what a girl is), and Oolong drugging Bulma (he gets his comeuppance from it, so the joke is on him, and earlier in the arc Bulma also tried to drug and rob Oolong)... But then early Roshi is pretty bad. Early DB is still great but it does make it a little difficult to reccomend (its great! Just ignore <all this pervy stuff>, it calms down after the first arc! Uh... Mostly...)

It was toned down in the anime compared to the manga, but yes, the dub had to rework it. They even had to change Sakura's mother's age to be a "college" student instead of 16 because a college student marrying her teacher has no other implications to it whatsoever lmao.
On the flipside of the dub coin when it comes to aging up characters to hide uncomfortable truths, the dub for Tamers accidentally implied Rika's mother had her at 15 instead of 18 because they aged up the kids in the script.
