I don't understand how reaction score works. Does it go up when you get a reaction, or are there good and bad reactions?

Yes it does, and yes there are. I don't know the mappings myself (obviously some things can be assumed, score go down for top hat and autismo ratings, score go up for medal/green checkmark), but the whole system isn't documented somewhere besides the bowels of Null's Xenforo config.
@CatParty's greatest idea, back when he was still online, was that we shuffle the score values on a monthly basis to out the people obsessing over it when suddenly they're sprinkling the deviant rating everywhere to give good karma.

I have said it before to other people who don't understand stickers and I will say it again, the reaction score does not matter. If there is a sticker worth receiving its an Informative sticker. You can have 50 neg rates and your post will be highlighted.

Can someone explain the rating system to me? I assume Winner is positive and Dumb is negative, but what about Horrifying and Lunacy for example? Usually they are a reaction to the post, not a commentary on the post's quality, so are they just neutral?

HHH is the least involved in the League of Shitty Lunatic Copecage T&H Enjoyers, and he likes sports. He might be a complete lunatic over blompf and vaccines but at least he contributes some drama and usually his posts create some funny conflict, unlike the useless reaction spam that contributes NOTHING.
As I said, reacting without posting, or moaning about shit without ever coming with YOUR alternative and positive vision is waste of compute power.
