Not a belly button, that’s the exit wound of a suprapubic catheter, e.g. a catheter that is drilled directly into the front of your bladder instead of being inserted up the peehole.

Currently nearly 3 years post op from Stage 1 Phalloplasty with Hans/Johannes Goossen at Greenslopes private hospital in Brisbane, Australia.
Stage 1 19th August 2018 - 7 night hospital stay* - 10 hours in surgery**:
Pedicle thigh graft to create Phallus
Split thickness skin graft (to cover phalloplasty graft site)
Neo urethra creation (within the phallus)

Stage 2 14th February 2019 - 5 night hospital stay* - 8 hours in surgery**:
Urethral Hook-up
Scrotoplasty
Vaginectomy
Glans creation
Clitoral burial (opted for this, but you don’t have to)

Stage 3 29th August 2019 - 2 night hospital stay* - 6 hours in surgery**
Testicle implant (silicone, left side)
Erectile device implant (pump right side testicle, reservoir inside the pelvic cavity, both inflation rods used) https://www.hardfacts.com.au/treatment-options/penile-implants.html same device used for CIS men with ED

*Hospital stays are unique, this was my personal hospital stay time frame*
*Surgery times are also unique, this was my personal experience (typically these surgeries are shorter)

Complications:

Complications following Stage 1:
Hematoma underside at mid shaft
Fistula underside base of shaft
Mild infection on thigh graft site
All just required dressing changes and resolved themselves. Mild infection took another round of antibiotics and Silver dressings over the infection site which resolved the issue.

Complications following Stage 2: Luckily no real complications following stage 2! My glans site healed a little too well, which is why the 'head'/glans does not have a very mushroom-like appearance. Glans revision was possible, however I opted out as it would have needed to be completed prior to stage 3, pushing the last operation out another 6 months with the possibility of the same thing happening again.

Complications following Stage 3:
Small opening of the left and right side incision sites- required dressings but resolved themselves)
Contracture capsulation of the right testicle (housing the erectile device pump) - required some self administered massage of the area daily for a few months - now resolved.
Left side testicle further back than right side due to scar tissue (again this could be revised, but I opted not to)
There is a noticeable bend in the phallus, this is partially due to the weight of the phallus, some contracture capsulation around the inflation rods and also the scar tissue along the underside of the phallus. This doesn’t hinder my ability to have penetrative sex or have any other complications or pain at all.

Other surgery notes:

Length is 5 ½ inches/14cm
Circumference is 6 inches/15.5cm

Just for reference, ALT produces a girthy penis. No regular condoms fit this size, a 69mm condom is snug (you can’t buy them off the shelf here in Australia, you have to buy them online).I am very happy with my result and even though things didn’t go 100% to plan and I was a little disappointed with a couple of things (glans scar flattening, bend in the penis) overall I feel lucky to have this result. Sensation was not at the top of my priority list, ALT doesn’t always produce the best sensation so I was aware of this. Sensation is okay, I can orgasm during sex and from hand jobs, however not from oral sex (this is equally due to the size of the phallus and the sensation). I can feel the phalus all over, but it is not the same tactile feeling as touching another limb. Orgasm is mainly possible due to the buried anatomy that is stimulated during sex. Again, just my experience as not everyone has the same result. Urination is okay. Due to some narrowing and roughness in the neo-urethra my stream is sometimes unpredictable! For this reason using urinals/troughs is A++, but peeing standing up over a toilet can be precarious. I’ll never complain about CIS men not being able to aim again. Overall i'm happy with this result, public urination was a main concern and cause for anxiety, sitting to urinate at home no longer gives me dysphoria because I know I have the option to stand if I want to.

My experience with Dr Goossen (pronounced Goh-sen) was awesome. Hans is an excellent surgeon and person. He is risk adverse because he cares about the outcome of the surgery and the wellbeing of his patients. If you are considering ALT, Hans will tell you whether this is a viable option for your body type, the leaner (lower body fat %) the better.

Stage 1- Abdominal/pubic phalloplasty

Stage 3- Urethral hookup, scrotoplasty and Glansplasty

Pubic phalloplasty with Hans Goossen in Brisbane QLD.

1st surgery- creation of phallus, essentially mini abdominoplasty. 6 days in hospital, I went home and back to work on light duties from 3 weeks post op.

2nd surgery- creation of urethra using forearm flap. Much smaller than RFF and nerve hookup. No complications except nerve pain in donor arm, sensation still coming through in penis @6 months post op but very happy so far.

3rd surgery- hookup urethra, glansplasty, scrotoplasty (no implants), vaginectomy and debulking at the base. Still healing from this stage so can't comment on anything other than aesthetics. Very happy so far.

estimated price (100k) includes cost of final stage (erectile device and implants), interstate travel and accomodation, laser hair removal, OT visits, consult costs, private health excess's, medications, dressings, scans, tests etc etc. I'm not one to keep a spread sheet but everything adds up.
