This is a question I don't see asked often enough. It's a raging debate whether children can consent to SRS, HRT, or puberty blockers (it shouldn't be, it's obvious to anyone who isn't a pedophile that they can't). But a question that isn't asked enough is: Can adults consent to it?

Spoiler: Further explanation and my opinion

My answer to this question would be no. Absolutely not.

Trooning out involves permanently forfeiting your fuckability (except to pornsick chasers), your fertility, and an authentic life as your biological sex in favor of a crude mockery of the opposite sex, or some subhuman blob in between. So why sacrifice so much in favor of living such a cursed, miserable life? Why sacrifice so much in a sad attempt to become something that isn't even your authentic self?

The obvious answer is that such people are too mentally ill to actually consider the costs, or even care about them. And when they groom children, they go out of their way to avoid telling kids about the horrors that come with trooning out.

It's safe to say that there is not a single person who would consent to such operations, that actually has the mental competence to actually consent. If they had the ability to consent one way or another, the answer would always be no. Treat the underlying mental illnesses, and the desire to transition also dies with it.

I think that it’s not so much about adults consenting to trooning out, but what kind of people these are in general. As we have seen countless times (and even in your explanation), it’s mostly pornsick coomers and consoomers who troon out (for TiM) and for TiW it’s mostly aidens jaydens and haydens who have been groomed by consuming yaoi/anime content (kind of like how most TiM absorb and frantically view pornography).
Personally, I don’t think they can consent to trooning out. For many of these people, they are too far gone mentally (usually autistic hyperfixation) and there are countless threads of people on this site who shouldn’t have gone through with transitioning. Kevin is a good example, and now that he’s gone through SRS he copes far too often on Twitter - he clearly is not happy. A lot of these people aren’t happy. They should have sought out mental help, instead of having their “egg cracked”. Look at the people Kevin associates with on Twitter and the people in his “polycule”. They’re all miserable and cope through their disgusting fetishes (i.e: Jen eating literal shit).

We generally do not treat adults who believe things contrary to objective reality as competent or able to grant consent. I see no reason this should not apply equally to those who believe they are the opposite sex.

People who have developed gender dysphoria, likely due to psychological or sexual abuse IRL or through the media at a young age, should be counseled to accept the sex they were born as.

If a person thinks they can fly, we usually don't let them jump off a building, If some asshole thinks that they're Teddy Roosevelt we don't start calling them Teddy, advise them to get surgery to look more like 'how they really look on the inside', and let them run the country.

If a person is unable to be counseled to accept their actual sex, and if they are a potential danger to themselves (may attempt trooning out) or others (may tell children this is normal) they should be put in a mental institution.

I also include the right to suicide in my OP. I don't know what your ideal world would look like but I assume it's one where people that want to Troon out are institutionalized at the expense of the taxpayer.

I don't have an answer to this question, because I really don't care much for rights discussions.

However, here's some food for thought. If a young healthy early 20s person wants to get the snip/tube tie they will struggle to find a doctor that will perform the surgery because "they might live to regret it" or "they're not mature enough to know that's what they want." However, that same person, or even someone younger, can find plenty of doctors to run them through a meat grinder if they claim gender dysphoria. So I don't know if people OUGHT to have the right to transition, but if you compare transitioning to any other medical procedure it is treated in a way that flies in the face of contemporary medical ethics.

Your point about not giving surgeries to people clearly suffering from dysmorphia is another good example. Trooning out is an aberration of the medical world in terms of how it's handled.

Isn't that logic how we got into this mess in the first place? We kept giving inch by inch under the pretense it was to just help poor downtrodden trannys, and now suddenly they're in the position of power and demanding to give experimental hormones to children. I like the idea of live and let live, but experience indicates it never works out.

Consent implies informed consent, which is usually not available to these people because doctors who warn patients of the potential permanent damage to their mental health, hormones, and bodies from these medications and surgeries get called transphobic in online circles and are avoided by anyone seeking treatment in the future.
