There's actually not as much varience between head sizes as you'd think. Most average adults have a head circumference of ~22-24 inches. Children aren't too much smaller, a child of only 7-8 has a head that's about 18 inches around. Women don't necessarily have smaller heads either, my husband and I both have a 22 inch head.

I suspect that commercial hats tend to fit a 22" head as that's probably the average size, or at least the average unisex size. I can't wear one-size-fits-all women's knit gloves because I have long fingers. Mass marketed fashion isn't meant for outliers, even outliers that don't fall much outside of the norms.
