Major oversight? This just sounds like typical Indian SOP.

It doesn't even seem like they cut power to the machinery unless that was capacitance still discharging.

Speaking of oversight, all those people just standing around in that industrial accident smoke.

It looked like they were using a CO2 extinguisher to me. The problem was the fire was too big and drawing too much current. They should've tried...
