They look like fingerprint (or whatever) smudges on the scanner itself used to digitize the document. That's why they're in roughly the same spot...

If it’s legit, then my guess is it’s a hard copy of the document that’s been scanned to pdf.

It depends on what scanner was used (and what copier printed the original copy), but if there are any smudges or debris on glass or roller, that...

The toner cartridge needs to be replaced. The same thing happens to with my printer when I have a bad toner cartridge, usually a generic one.
