I think at some point, usage of that card stopped being "accidental", and more "how long can I keep this up before somebody says anything?"...

I was on a startup where that happened, one of the co-founders did it after his gf (ugly as fuck too) convinced him to splurge using the company...

One theory about that I read is that he was confronted with a suspicious transaction and thought he was only confronted for one transaction...
