Because he balanced out this more "distant" evil with being a terrifyingly prolific serial killer and rapist, complete with a rape/torture dungeon uncovered a few years ago. Using his utter impunity he would regularly abduct and rape young teenage girls, as well as blackmailing women with threats to send them and their relatives to the gulags to ensure their silence. Any who resisted were murdered and their bodies either dumped in the aforementioned dungeon or buried in his garden.

Apparently his actions were so infamous within the Soviet government that Stalin tried to keep his daughter as far away from him as possible.

A Soviet serial killer that murdered an estimate of 56 people. He would murder his victims then masturbate onto the bodies. The only way for him to get an erection was from murder. During the trial, Chikatilo was kept in an iron cage in a corner of the courtroom to protect him from attack by the many enraged and, often, hysterical relatives of his victims. Chikatilo repeatedly interrupted the trial, exposing himself, singing, and refusing to answer questions put to him by the judge.

He killed men, women, boys and girls all equally. He didn't give a shit who he killed so long as he was able to get off afterwards.

Around 250 murders have been ascribed to him making him the single most prolific serial killer in British legal history. His modus operandi was to inject his elderly patients with diamorphine and then forge their medical records in order to make it out that they were in poor health. He was only caught after the daughter of his last victim was informed of her mother's will, which made out a significant amount of money to Harold Shipman and was very likely forged. He was tried for 15 murders but the number that he actually committed is likely to be much higher. He managed to kill himself once he was locked away in prison, and his motive for killing all his elderly patients will never be known.
