The marks are probably from him re-siting his insulin pump. The site needs to be changed every few days and there's often some local irritation either from the insulin itself or from the adhesive. Some people need to change sites more frequently because of that irritation (if scarring develops, it can mean that area can't be used in the future - which is a big problem when you'll have to find sites for your entire life).

People with Type 1 can be overweight. Insulin helps you use food more efficiently but it doesn't stop you from overeating. You still need to control your food intake, and especially your carb intake. Plenty of diabetics are slack about that, especially young ones who haven't really developed any complications from poor management.
