You mostly have the wrong end of the stick about the book. The fertile women do want to have kids, most of the women we get to know anything about were already mothers before the Gilead revolution. That's how they are known to be fertile. The main character was a married mother but her husband was most likely killed and their daughter given to a different family. All of the fertile women are grieving that they don't get to raise their own children. At the same time there is a heavy implication in the book that the fertility problem has as much if not more to do with sperm counts, so the solution to the fertility crises is one that ignores a major issue as most proven fertile men were murdered for trying to defend their families.

Atwood wrote the book about the Iranian Revolution while adding in existing 80s fears about pollution leading to infertility as a catalyst for a similar revolution in America. The Handmaid idea comes from Bilhah in the Book of Genesis as she wanted to do an 'It Could Happen Here' style story, and this was her way of justifying a Christian society acting in a somewhat Islamic way, ignoring the inherent differences between Christianity and Islam. It also gave her an excuse to add rape to the book and while it's nowhere near as constantly gratuitous as it is in the tv series, I think it's fair to say that there is definitely an element of sexual titillation to a cohort of readers that are specifically reading the book for that reason. I don't believe Atwood was trying to state that there was ever any risk of an American Gilead but was mainly trying to raise sympathy for women in Iran.

There are huge differences between the book and the tv show. The book, while flawed, is not like the tv show. The Offred character is not ever any sort of girlboss. She's partly quietly accepting of the situation she's in, not wanting or enjoying it, but feeling that she wants to survive and chooses living this way over an alternative that would kill her. She takes pleasure in the small things she does have, she gets emotionally drawn into the ways of Gilead, is enthusiastic about particicution (state mandated executions carried out by handmaids), envies pregnant handmaids and values the lives of all babies. She, and the book, are critical of second generation feminism, with the narrators mother being a feminist single mother which the narrator experienced as flawed and hypocritical. It's a much more complicated and nuanced story than the tv show portrays.
