The reason people (especially men) buy OF isn’t just for the porn, it’s either because they’re lonely and want to feel like they have some guaranteed way to talk to women, or because they have some weird fetish and they need to hyper specific content to fulfill it that’s only locked behind a paywall. I think it's just top-down lighting and the shadow of her dough-faced pearhead.

Edit: Looking more closely at the shadows, there is an overhead light, but there also appear to be two offset front lights, probably on either side of the mirror. So it's probably the shadow of the bra being cast by the light to the left in the image.

Edit: The great thing about digital photography is that you can immediately look at your pictures and see that you look like a cross-eyed retard because you're staring at your phone inches from your face again rather than looking at the camera and that you're casing a rectangular shadow across the middle of the stupid expression on your face and laugh and then take a better picture. And then I am beset by the creeping horror that, what if... what if these are the better pictures?
