The consequence of not having irl friends. Which in turn is itself fueled by being too insufferable to have irl friends.
It's sad because stable relationships with others would do her good. But it's going to be impossible to keep them up and running due to her general attitude. So unless Pixy changes nothing will change for her. I wouldn't want to be her friend. I'd be afraid of a meltdown over the least little thing. If she actually tried to look masculine maybe she could find some LGBT friends. But then you have to put up with the whole "I'm an intersex man with a penis in my stomach. It's urgent!".
I have nothing. All the obvious advice is advice she'd never take. I wonder what her gay brother thinks of all this.

Also any friends she might make in the local LGBTQ community would be at risk of her blowing up at them any time they offered her advice and constructive feedback. Friendship isn't just giving someone asspats, friends try to help each other and give each other advice. Something PT has a long history of reacting poorly to.

Pixi is the definition of beauty parlor. Fat but sometimes thin, posts gravure then wants to be a man, artsy but incompetent, whorish but pure, can't keep a job or friends, strong but weak, changes identify constantly, broke but spends, family hates her but then gets charged for assault against family.

You can't make someone with mental health problems get help if they don't want it. The only way she'd get put in a hospital would be if she was an accrual harm to herself or others.

Where is she getting all these bad haircuts? If it's not a DIY and she paid for this then she deserves a refund. It's still not masculine though. It's like a fat soccer mom didn't have time to do her hair because she's got six kids to drive to school.
The skinwalking goes even deeper. Those blue eyes she used to hate because they weren't Nipponese enough are now an asset since they are just like Sora's.
